CUDWORTH Jack (Phil Butler) - July 31, 2010 in L.G.I. and of Wrenthorpe, Wakefield, West Yorkshire (formerly of Dublin). The loving husband of Jean, much loved dad of Rebecca, Simon and Sarah, beloved grandad and great grandad and a dear brother of Thelma and Pauline. Funeral service and cremation for Jack will now take place at Wakefield Crematorium on Friday (August 6) at 11am. Family flowers only please, donations in lieu if desired to Parkinson's UK for which a box will be provided.
