CUSACK (Jack) (Rathgar, Dublin and formerly of the Cosy Bar, The Coombe) - April 20, 2005, (peacefully) at St James's Hospital, beloved husband of Eileen; he will be very sadly missed by his loving wife, daughters Patricia, Marese and Bernadine, son David, brother Denis, daughter-in-law, sons-in-law, grandchildren, great grandchildren, sister-in-law, brother-in- law, nieces, nephews, relatives, neighbours and friends. Rest in peace. Removal this (Friday) evening from Fanagan's Funeral Home, Aungier Street to Church of St Nicholas of Myra, Francis Street arriving at 5.30 o'clock Funeral tomorrow (Saturday) after 11 o'clock Mass to Mount Jerome Cemetery. Family flowers only, donations in lieu to Our Lady's Hospice, Harold's Cross.
