CUSACK (Dr.Loman) (Sandymount, late of Goatstown, Dublin and Carrigallen, Co. Leitrim) - April 21, 2009 after a short illness in the wonderful care of the doctors, nurses and staff of St. Vincent's University Hospital, Elm Park, beloved husband and best friend of Pauline, loving father of Ann, Michele, Loman and Adrienne and brother of Mella, Matt, Brona, JEF, Barry and the late Fintan and Una; deeply regretted by his loving wife, son and daughters, brothers and sisters, daughter-in-law Ria, sons-in-law Peter and Alan, grandchildren Petey, Sive, Loman and Senan, brothers-in-law, sisters-in-law, nephews, nieces, extended family, colleagues, patients and friends. May he rest in peace. Removal this Wednesday evening from St. Vincent's University Hospital, Elm Park at 5.45 o'clock to the Church of the Sacred Heart, Donnybrook arriving at 6 o'clock Funeral tomorrow (Thursday) after 10 o'clock Mass to Dean's Grange Cemetery. No flowers please.
